After completing the examination:

– You will be asked to pay the full cost of treatment.

– Dong Nai -2 Hospital will provide necessary documents and medical records for you to submit a claim to the Insurance company.

Outpatient treatment

– Request form for settlement of Insurance benefits (signed by the insured person)

– Valid payment invoice: in case of an electronic invoice, it is necessary to add a one-time conversion invoice from the electronic invoice Detailed list of hospital expenses Medical report Test and diagnosis results Imaging diagnosis (if any) Prescription: clearly stating the diagnosis

Inpatient treatment

– Request form for settlement of Insurance benefits (signed by the insured person)

– Valid payment invoice: if it is an electronic invoice, it is necessary to add a one-time conversion invoice from the electronic invoice.

– Detailed list of hospital fees

– Medical history report

– Paraclinical results (if any)

– Hospital discharge certificate/Discharge summary/Surgery certificate (if any)

– Indications for follow-up examination (if any)

III. Accident treatment
